New Registrations

The doctors welcome new patients who live within our practice postcode areas which are;

• DN1 3

• DN4 8, DN4 9, DN4 0

• DN11 9 (Loversall and Wadworth only)

If you wish to register, please complete a registration form and a new patient questionnaire, which you can find below. It is important that you read these documents carefully and fill them out the best you can. Medical treatment is available from the date of registration, which can take up-to 2 weeks.  Please contact reception for further information.
